Living with Chronic conditions doesn’t mean giving up the life you enjoy. The Chronic Disease Self-Management Program (CDSMP), developed by Stanford University, is a proven program that helps older adults take greater control of their health and well-being.

If you are living with a chronic condition, regardless of how simple or complex it may be, this workshop is for you. CDSMP provides tools and support to better manage symptoms, handle day-to-day challenges, and make healthier choices.

Through engaging group sessions, you’ll connect with others who share similar experiences while learning practical strategies to build confidence, stay active, and improve your quality of life.
